Who Qualifies for Awake Body Contouring

Candidacy is decided at examination, not by a BMI number. Stable weight matters more than reaching a goal weight. Dr. Zelko performs the assessment himself and will tell you directly if body contouring is not the right answer.

Dr. Timothy Zelko: The Consultation Promise

“The consultation exists to answer one question honestly: can I give you what you’re actually looking for? If the answer is yes, we talk about how. If the answer is not yet, we talk about what needs to happen first. If the answer is no, for any reason, I tell you that directly, explain why, and point you somewhere that can help you better than I can. What I will never do is take your money for a procedure I don’t believe will give you what you came for.”

Dr. Timothy Zelko, D.O.

“Am I a good candidate?” is really two questions. The surface question is whether you qualify medically. The deeper question is whether you deserve to do this for yourself. Both get answered honestly in the consultation.

The Ideal Guest

She Already Knows What She Needs

Most guests who find their way to Zelko Aesthetics have already done the work, years of it. Dieting, exercise, everything the internet recommends and everything their doctor suggested. They are not here because they gave up. They are here because they figured out that surgery is the only tool that addresses what pregnancy, C-sections, and time have done to their bodies, and no amount of effort at the gym is going to change that.

That clarity, that combination of self-knowledge and realistic expectation, is the most important candidacy factor Dr. Timothy Zelko looks for. It is not a number on a scale or a measurement on an exam. It is a mindset.

Who She Typically Is

35 to 55. Done with childbearing, or no plans to have children. A professional, someone with responsibilities at work and at home who cannot disappear for weeks. She pays her own way and makes her own decisions. She wants this done right, done efficiently, and done without general anesthesia.

What She Actually Wants

Not just a flatter stomach. She wants to step out of the shower and genuinely like, or love, the person looking back at her in the mirror. She wants to feel like herself again. Once that mirror moment happens, everything changes: longevity becomes a real goal, health span becomes something worth pursuing.

The Abdomen After Pregnancy

The most common presentation: significant weight gained and lost through pregnancy, leaving skin that no longer retracts. Or a C-section scar that creates a ledge for skin to drape over as time passes. Both are surgical problems. Neither responds to diet or exercise. Both are exactly what Dr. Timothy Zelko addresses.

Documented Outcomes: What Guests Actually Experience

The two most common reasons women delay procedures they have wanted for years are fear of pain and fear of downtime. For procedures performed under Dr. Timothy Zelko’s protocol, both fears are consistently larger than the reality. Pain during all procedures averages 0 to 2 out of 10, guests who prepared for a 7 or 8 describe the actual experience at 0 to 2. Return to desk work is routine at 1 to 2 days after TickleTite and 4 to 5 days after a TickleTuck. These are not best-case figures. They are documented, routine outcomes across hundreds of procedures.

The Mirror Test, A Self-Assessment

“When I look in the mirror, am I bothered by specific areas that are out of proportion with the rest of my body, or am I bothered by my overall body size?”

Take the Mirror Test

If the answer is specific areas, your abdomen, your arms, the deposits that haven’t moved despite years of effort, that is a distribution and shape problem. Body contouring addresses it directly. If the answer is overall size, that is a weight management situation and a different conversation entirely.

“They want to step out of the shower, look in the mirror, and really like, or genuinely love, the person looking back. They want to appreciate themselves again. And once that happens, once that mirror moment is real, longevity and health span become realistic goals. The surgery isn’t the end. It’s the beginning.”

Weight & Body Composition

No Fixed Cutoff: An Honest Conversation

There is no BMI number that automatically disqualifies a guest at Zelko Aesthetics. Larger guests are welcomed and operated on regularly. What is non-negotiable is alignment: the expectation has to match what the procedure can actually deliver at your current weight.

A TickleLipo or TickleTuck on a guest with a BMI of 35 or above will produce a meaningful, real improvement. It will not produce a bikini-ready result by the following weekend. Dr. Timothy Zelko will tell you exactly what is achievable for your specific body at your current weight, and if your expectation differs from that, the conversation continues until it doesn’t, or the procedure does not happen.

Path One

Continue the Journey: Then Return

If you are currently losing weight and making real, sustained progress, the honest recommendation is often to continue. Operating at or near your goal weight produces a more accurate skin excision, more meaningful liposuction, and a more complete result. Returning after reaching your target produces a better outcome than operating now.

Path Two

Proceed Now: With Full Disclosure

For the guest who has genuinely exhausted every option, hit a wall, and lost momentum entirely, a procedure may be exactly what restarts the engine. Full disclosure of what the result will and won’t look like at your current weight. Sometimes a person needs a win to believe another win is possible. The mirror moment becomes the motivation. The surgery becomes the jumping-off point.

Medical History

How Dr. Timothy Zelko Actually Thinks About It

The medical history review at Zelko Aesthetics is not a liability checklist. It is a genuine clinical assessment of whether your body is positioned to heal well and respond safely to an in-office procedure under local anesthesia. Most conditions are evaluated individually. A small number are absolute stops regardless of circumstance.

ConditionStatusHow It Is Evaluated
Well-Controlled DiabetesGenerally acceptableAcceptable with a good HbA1c and medical clearance from your primary care physician. Well-controlled means well-controlled, not “mostly managed.”
Uncontrolled DiabetesHard stopHard stop. Uncontrolled blood sugar impairs every stage of wound healing. Not a discussion, not a borderline case.
Autoimmune DiseaseIndividual assessmentAssessed individually. Specific diagnosis, severity, prior healing history, and current medications all matter. Primary care clearance required. Some presentations are workable. Others are not.
Anticoagulation TherapyIndividual assessmentNot an automatic disqualifier. Assessed by: what you are on it for, whether you can safely stop for a defined window, and approval from your managing physician. Dr. Timothy Zelko has operated on anticoagulated guests, carefully and with appropriate coordination.
Significant Cardiac DiseaseHard stopHard stop. The cardiovascular demands of any surgical procedure are not appropriate for guests with significant cardiac compromise.
Significant Kidney DiseaseHard stopHard stop. Renal function affects medication clearance, fluid balance, and the body’s capacity to manage surgical demands.
Immunosuppressive TherapyHard stopAbsolute hard stop. No exceptions. Immunosuppression at any level fundamentally compromises healing and infection resistance for elective surgical procedures.
Active Infection: AnywhereHard stopHard stop until fully resolved. Surgical trauma is not added while the body is already fighting an infection, regardless of location. Reschedule once resolved.
Previous Significant Post-Op ComplicationsHard stopHard stop for further evaluation. A history of severe infection, significant wound breakdown, or serious complications tells Dr. Zelko important information about how your body responds to surgical trauma.

Smoking & Vaping

The Ask Is About Your Result

Smoking impairs healing in specific, documented ways. The restriction exists because Dr. Timothy Zelko wants your outcome to be the best it can be, not because of judgment about your habits.

Heavy Smoker, >1 pack / day
●
Hard stop. The vascular compromise at this level of nicotine exposure makes wound healing unpredictably problematic. Not a reduction conversation, a candidacy conversation.
Moderate Smoker, up to 1 pack / day
◐
Acceptable with a genuine reduction to ten cigarettes per day or fewer before the procedure. Full cessation is strongly preferred. The closer to zero, the better your result.
Vaping
◐
Treated the same as moderate smoking. Nicotine is nicotine regardless of delivery method, the vasoconstriction effect on healing tissue is identical. Same reduction ask applies.

Why Smoking Impairs Healing: The Short Version

Nicotine causes vasoconstriction, it narrows the small blood vessels responsible for delivering oxygen and nutrients to healing tissue. Reduced perfusion means slower cellular repair, higher infection risk, increased wound breakdown risk, and a less refined scar.

Carbon monoxide from cigarette smoke compounds this by binding to hemoglobin more readily than oxygen, so even the blood that reaches the tissue is carrying less of what the tissue needs to heal. Even a meaningful reduction in smoking, not full cessation, improves tissue perfusion enough to produce a measurably better result.

Prior Surgery in the Area

Previous Procedures: Usually Not a Problem

A history of prior surgery in the abdominal area does not concern Dr. Timothy Zelko in itself. A significant portion of guests presenting to Zelko Aesthetics are there because Dr. Zelko is correcting what a previous surgeon’s procedure left behind. The history of a prior procedure does not change the willingness to operate. What matters is the healing history, the timing, and what the tissue actually looks like now.

Recent Surgery

<3 Months — Wait

The tissue needs time to fully settle, soften, and re-vascularize before returning to the same area. Operating within three months of a prior procedure in the same region compromises both safety and result quality.

3–6 Months

Possible, evaluated individually

Three months is the minimum threshold. Six months or more is preferred. The longer the tissue has matured and stabilized, the more accurate the assessment and the better the healing response.

6+ Months

Generally acceptable

Tissue is mature, vascularization re-established, and the anatomy can be assessed accurately. Prior scars in the area are often incorporated, removed and replaced with a cleaner, better-positioned result.

Previous Scarring: Often an Opportunity

Dr. Timothy Zelko frequently incorporates prior scars into the excision plan, removing the old scar and creating a new one that heals better, sits in a better position, and looks significantly more refined. Old scar removal is not a complication of the plan. It is often part of it.

Expectations

If We Can’t Align on the Outcome, We Don’t Proceed

The expectation conversation is the most important conversation at the consultation. It is not a formality. It is the entire point. Dr. Timothy Zelko needs to know what you are expecting to receive, and you need to know honestly what he can deliver. If those two things don’t match, the procedure does not happen. There are three versions of this conversation.

The Misaligned but Educable Guest

Her expectation doesn’t match the deliverable, but the gap exists because nobody has ever explained to her what a realistic cosmetic surgery outcome actually looks like. This guest gets educated. Not dismissed, genuinely taught. What is an excellent result for her specific presentation. What the procedure addresses and what it does not. Many guests in this category realign their expectations and become excellent candidates. Some don’t, and they self-select out honestly, which is better for everyone.

The Guest Pursuing Something Unreasonable

What she wants is not achievable by any surgeon through any procedure. Dr. Timothy Zelko refers her to other physicians who may be better positioned to help, or continues to educate her on what any cosmetic procedure can realistically accomplish. She leaves knowing more than she arrived with, regardless of whether she becomes a guest.

The Guest Who Will Never Be Happy

This guest is identified behaviorally, not clinically. Multiple procedures. Multiple surgeons. A pattern of dissatisfaction that follows her from practice to practice. She speaks negatively about every previous physician she has seen. This guest is declined care, and told directly and honestly why. A surgeon who accepts a guest he can already identify as someone who will not be satisfied regardless of outcome is not serving that guest. He is serving himself. That is not the practice Dr. Timothy Zelko runs.

Quick Reference

Hard Stops and “Not Yet” Situations

Absolute Hard Stops: These Do Not Proceed

  • Uncontrolled diabetes
  • Significant cardiac disease
  • Significant kidney disease
  • Immunosuppressive therapy, any level
  • Active infection anywhere in the body

Frequently Asked Questions

What Guests Ask Before the Consultation

I haven’t had children, am I still a candidate?

Yes. Childbearing is a common reason guests present with the anatomy these procedures address, but it is not a requirement. Significant weight fluctuation, genetics, aging, and time all produce laxity and fat distribution changes that surgery can address. Candidacy is determined by your anatomy and your goals, not your reproductive history.

I had a previous tummy tuck and I’m still not happy with my upper abdomen. Am I a candidate?

Possibly yes, and this is one of the three primary presentations the Reverse TickleTuck addresses specifically. If a previous abdominoplasty repositioned your belly button and removed lower skin but left the upper abdomen unresolved, the Reverse TickleTuck addresses the upper abdomen through a completely separate incision in the inframammary fold, without revisiting the original lower scar. Timing matters: if the previous procedure was less than 6 months ago, the tissue needs more time to mature before Dr. Timothy Zelko can accurately assess and plan.

I’m on blood pressure medication. Does that affect my candidacy?

Blood pressure medication alone does not disqualify you. Well-controlled hypertension with a stable medication regimen and clearance from your primary care physician is generally acceptable. What matters is that the underlying blood pressure is controlled, not that you are on medication to achieve that control. Bring your current medication list to the consultation and Dr. Timothy Zelko will review it directly.

I plan to have children in the future. Should I wait?

For TickleTuck specifically: the standard recommendation is yes. Pregnancy after a tummy tuck will stretch the skin that was tightened, potentially requiring revision. If there is a meaningful chance of future pregnancy, waiting is the conventional guidance, and Dr. Timothy Zelko will have that conversation directly at every TickleTuck consultation where it is relevant. For TickleLipo alone, timing relative to future pregnancies is less critical, since liposuction does not involve skin excision or muscle repair.

I’ve had liposuction elsewhere and wasn’t happy with the result. Can Dr. Zelko fix it?

Often yes, and this is a common presentation at Zelko Aesthetics. A significant portion of guests are there specifically because Dr. Timothy Zelko is addressing what a prior surgeon’s procedure left behind. The history of a previous liposuction does not change willingness to operate. What matters is the timing (minimum 3 to 6 months after the prior procedure, 6 or more preferred), what the tissue actually looks like now, and what a realistic correction looks like for your specific situation. That assessment happens at consultation.

What if I’m not sure what procedure I need?

That is the most common place guests arrive, and the consultation is exactly where that question gets answered. The decision between TickleLipo only, TickleTite (TickleLipo + Renuvion), and TickleTuck requires a physical examination. Dr. Timothy Zelko measures your laxity, assesses your fat distribution, discusses your goals, and tells you directly which procedure or combination addresses what you are describing. Many guests come in thinking they need a tuck and leave with a TickleTite plan. Others come in hoping for TickleLipo alone and leave understanding that a tuck is the honest answer. The examination resolves the question that photos and descriptions cannot.
